Financial Flexibility for Patients
The financial burden of cancer care can be overwhelming. Caris offers programs that provide financial flexibility to qualified patients. The Caris Financial Assistance Program can assist uninsured patients or those unable to afford associated out-of-pocket testing costs if program criteria are met.

Working with Your Insurance
Caris testing is typically reimbursed by insurance providers, Medicare and state Medicaid providers. The cost will differ among the Caris offerings, depending on the tumor profile offered, and the number of biomarkers analyzed and technologies used. We will perform the services ordered and deemed medically necessary by a physician, regardless of our network status.
A Test is Ordered
At the time of order, all necessary health insurance information is required for every patient. Caris will file insurance claims for services provided on the patient’s behalf.
Caris Submits a Claim
If coverage is denied, Caris billing specialists will work with the insurance provider to file appeals and pursue coverage on your behalf.
Billing
Caris will bill your insurance company directly and work with them to process claims. Depending on the terms of your healthcare plan, you may receive a bill for the deductible, co-insurance, or co-payment as determined by your plan.
Patient Billing FAQs
Each time Caris submits a claim or an appeal to your insurance provider, the insurer will send an EOB that details the portion of the claim the insurer intends to pay. Some insurance providers will split one claim into multiple sections, which may cause you to receive more than one EOB. Even though an EOB may state an “Amount Due.,” No payment is due until you receive a Caris invoice.If you have any questions about an EOB or an invoice from Caris, please contact Patient Billing at 1.888.979.8669 or PatientBilling@CarisLS.com.
Patients are financially responsible for deductibles, co-payments, and/or co-insurance according to the patient’s individual insurance plan and applicable federal and state regulations. Contact Patient Billing at 1.888.979.8669 or PatientBilling@CarisLS.com to discuss your options.
Caris will conduct any prior authorizations required by the patient’s insurance plan, and Caris will appeal all prior authorizations or claims that are denied. For questions, please contact Patient Billing at 1.888.979.8669 or PatientBilling@CarisLS.com.
Caris will file all secondary or supplemental insurance claims on your behalf. If you receive a bill and believe that a secondary claim has not been filed, contact Patient Billing at 1.888.979.8669 or PatientBilling@CarisLS.com.

If you receive a reimbursement check directly from your insurance provider, please forward those funds directly to Caris for the services we performed.
